While reviewing Mr.Ortege's health history form, you notice that he did not fill out the Social and Occupational History part of the form. Why might Mr.Ortege not have filled out this part of the form? What should you do?

Mr . Ortege might not have filled out the social and occupational history part of the form because he might be in a state of mind that if he fill this part of the form it might have a low profile image of him in eyes of the doctor if his social and occupational background is not of high standards . Also he might be in doubt about the confidentiality of his particulars.

In such case the nurse or doctor should talk to him and counsel him that this part of history is important for his diagnosis and for further treatment. Also the doctor or the nurse should assure him that all his information will be kept confidential and should not worry about his particulars.
