Medical records have many purposes in patient care , treatment, future care ,follow up and research

a) In a medical record, clinical disciplines cannot document their disagreements directly because being a legal document derogatory comment about other physicians or departments and colleagues should be avoided however, if there is disagreements occurs then care providers should note variances in a chart and gave proper explanation for which decision they carried out.

b) Medical records are essential for defense in malpractice cases. A proper medical record is a strong weapon in court against malpractice cases.

c) Medical records co ordinates services across disciplines in hospital like medical , surgical,orthopedics , insurance, billing and pharmacy

d) Medical records are compliance with regulations of many acts just like HIPAA(health insurance portability and accountability act)
