A 15-year-old female is admitted to the hospital with a concussion after a biking accident. During the admission assessment, the patient confides in the RN that she doesn’t wear a helmet when she rides her bike. A nursing care plan has been developed using the NANDA-I nursing diagnosis Deficient Knowledge related to insufficient information about hazards of biking as evidenced by patient statement, “I don’t wear a helmet when I bike.” The outcome: The patient will verbalize the importance of wearing a helmet when biking before discharge.

The 15 hyear old female is in her Teen age stage , where they merely listen to the advice of parents and elders. Rather trying to learn after the mistakes or mishappenings.

The following intervention with rationale can be done

  • Instruct him to wear helment which can prevent from head injury and save a life during accident
  • Stay alert- This prevents from getting diverted
  • Obeying traffic rules_ which forms safety for both her and the other drivers on the road
  • Four wheeler driving- She should drive as if she is driving a 4 wheeler .where she can't overtake another vehicle.   

The patient will be able to verbalize the importance of wearing a helmet

If the out come is not met

The plan of care for pain can be planned

But tis will not be needed when client realises the need for wearing the helmet
