Family Background
The Family resides at Makati City. Members of the household
includes the client J, a 24 year old housewife who gave birth to
her 4 week old baby girl , her husband, M, 26 years old and their 1
year old son. J and M are living together for 3 years.
The client, J is the one who takes care of their infant and
the newborn. Occasionally, they argue for some concerns (financial,
household chores) in the family. They are all catholic. Client J
stated that their family has a very close relationship.
Socio Economic Background
They are both elementary graduates. They are fish vendors but
when client J got pregnant with the second child, she stopped
working to take care of her children. They are earning Php 16,000
per month and according to her the basic necessities were being
met.
According to the client J, she decides for the family.
Sometimes, her mother visits her to help her in taking care of her
children. They are not so accustomed to joining
city programs but she claimed that they usually have regular
visits to the city Health Center for her child’s vaccination but
because she just gave birth, she was not able to bring her son and
newborn to the health center to have their vaccination. When she
was pregnant she went to the health center for her monthly prenatal
checkups. She regularly takes multivitamins and ferrous sulfate as
prescribed by the physician. She added that they value health so
much and that she’s aware of the services present in the health
center but was not able to make time to visit because of her
situation.
Home and Environment
In their environment, lots of children are playing on wet
flooded streets. Houses of neighbors are connected. Sidewalks are
narrowed and there are lots of passers. Neighbors are washing
clothes on their sidewalks. Their house is rented for Php 3,000
with one room. Made up of half cement and half wooden materials. No
kitchen and drainage facility is open. The toilet they use is
outside their house which is also used by their neighbours but it
is clean. It is 4 meters away from their house and they use pail in
their toilet. The housing area is congested and they do not have
recreational facilities.
She mentioned that the living space in the house is enough for
them since they are only 4 in the family. Although there are room
windows, they usually do not open them because of the smell of the
garbage beside their house. The garbage bins near their house
usually overfill, although there are garbage collectors. They do
not segregate their garbage and just dispose off the trash in the
garbage bin even if they know that it is overflowing.
The source of their electricity is from a jumper. MANILA WATER
COMPANY is the source of their water for cleaning, bathing, cooking
and drinking. She mentioned that they have OROCAN to store their
kitchen and cooking utensils. Upon house visit, used kitchen
utensils were being left in the sink and placed in a basin with
water. She claimed that since she’s the only one being left in the
house to do the chores, she sometimes neglect to keep the house
clean. Presence of cockroaches, mosquitoes and rats was also
observed. Empty plastic water bottles were also noticed.
Health Status of each Family member
Client J verbalized that she and her husband did not have any
past or present illnesses. Her father died due to hypertension.
Client M’s father and mother died due to stroke while Client J’s
mother had occasional bouts of high blood pressure and is taking
Amlodipine as her maintenance.
Client J does not have any vice nor history of hypertension
and diabetes. They do not have a specific diet but they have three
regular meals a day which is usually composed of rice and a viand,
occasionally they have fruits for dessert.
Client J’s OB score is G2, P2, T2, P0, A0, L2. She gave birth
via Normal Delivery. She had a healing medio lateral episiotomy
from giving birth 4 weeks ago. Client J is currently complaining of
headache and claims that the reason was her inadequate sleep. She
does not have time to go for a check up. She is currently
breastfeeding her newborn but claims that she’s having a hard time
to keep her on latch because of an inverted nipple. She claimed
that she breastfeed her 1 year old son for a month only because of
the same problem. She wanted to breastfeed her 4 week old newborn
up until 2 years old but she thinks that she might not be able to
do so because of her inverted nipple. She has no time to do some
exercise or make time for herself because of the circumstance. She
is focused on taking care of her children and sometimes she skipped
meals in order to tender to them and do some household
chores.
She makes sure that her children are well taken care of and
usually serve her 1 year old child with rice and fish. She
remembered that when her eldest child was 6 months old, he was
admitted to the hospital because of pneumonia. She tries to
breastfeed her 4 week old baby but sometimes she gives her formula
milk especially when her baby cannot properly breastfeed.
Based on the family health history, they do not have
communicable nor any non-communicable diseases.
