He says he feels pretty good except for the soreness on the right side of his abdomen". His vital signs are TPR 99.2^F( tympanic) - 78-19 BP 118/74. He states he is currently taking two 325-mg acetaminophen twice a day for the pain because the hydrocodone made him too sleepy. progress note documentation.

Answer : patient has complaint of soarness on the right side of the abdomen . Patient has complaint of pain and on acetaminophen 325 mg  twice . (pain is not tolerated yet) . Vital signs : Temperature shows slight rise and other vitals are stable.     No other signs and symptoms. 1) pain management is the first consideration  2) investigate  the causes of soarness of the right side of the abdomen and further management.
