Nursing Care Plan: for patient with Diabetes mellitus:

Assesment:

Subjective Data: Inceased thrust,frequent urination, extreme hunger, unexplained weight loss, fatigue, irritability

Objective Data: Dr/ Nur observe

Nursing Diagnosis: deficient fluid volume related to vomating, nausea, diarrhoea, sudden weight loss.

Goal: Maintain fluid volume

Planning and Implimentation :

  • comferatble position
  • monitor vital signs
  • asess histoory related to this vometing, frequent urination, nausea etc
  • monitor skin colour and moisture
  • maintain fluid intake at least 2500ml/day
  • adminster insulin injection
  • monitor effectiveness of insulin and glucose test every 30 -60 min
  • administer medicine as prescibed by doctor

Evolution: Monitor vital signs and urine output and glucose level
