weight=195lbs=88.45 kg

Height=5 feet 11 inhes=180.34

Body surface area(BSA)=Height(cms)xweight (kg)

After applying formula 180.34 x 88.45 3600

= square root of 4.430

=2.104 nearest 100=2.10

Drug ordered is 25mg

recommended dose is 3.7-18.5 mg/m2

we need to calculate the right dosage by multiplying BOdy surface area and recommended dose

2.10  x3.7=7.77 mg(nearest 10) =7.8

2.10 x 18.5=38.85 mg(nearest 10)=38.9

The ordered dose is 25 mg is in between the range of 7.7 mg and 38.85 mg
