S). Situation

This is nurse X looking after patient Y in haematology department, as patient is suffering from severe headache as an adverse reaction for blood transfusion

B) background

History of patient, when Y was brought to hospital and for what reason along with

Amount of blood transfused to patient and time taken during this, and headache started during the ongoing blood transfusion or later on

A) assessment

Looking after vitals of Y

Including:- temp

H.R.

R.R.

B.P.

Any colour change in skin, or any other problem, or something different from normal, if B.T. is going on, stop it for some time

R) recommendation

I would like you to come and see the patient, as Y can't bear his pain anymore,
