Why some patient received both Albuterol and Ipratropium as a Duoneb. Some will just get one but not the other?

Albuterol is a beta 2 adrenergic, it is used with caution in cardiac disease , cardiac arrythmia, hypertension, convulsive disorder. In such patient ipratropium alone is prescribed.

Ipratropium bromide can't be used in patient with glaucoma, history of hypersensitivity to atropine.
