Need assistance in writing a PES statement (Below I have provided information which was obtained during the assessment conducted using the Nutrition Care Process). Intervention would provide weight loss plan for client, if you have any recommendations regarding that please include that also. Thank you.
- Basic information: 56-year-old white male
- Anthropometric information: Height = 5'6", Weight= 171.5 lbs. Therefore, BMI = 27.7 which is considered overweight for height.
- Weight Goal: 165 lbs. (lose 5 lbs.)
- Weight history: has fluctuated a little over years, but has consistently stayed around 170 the last 5 years.
- Family weight history: father was overweight with co-morbidities such as hypertension and mother also considered overweight and diagnosed with type 2 diabetes.
- Other diets tried in the past: Cardiac diet
- Triggers to weight gain: Working out less than the occasional 3 times a week, not maintaining a certain eating pattern - occasionally consuming foods that lack nutritional benefits.
- Medications/supplement use: Takes Actos once a day along with a vitamin D supplement once a day
- Smoking/alcohol use: non-smoker (has never smoked), drinks only on Saturday night his night off, sometimes averages 5-6 beers in a sitting.
- Eating habits: (How often do you eat out?, How often do you eat sweets?, What do you drink?)
One night we had Greek salad with chicken, the next was spaghetti, then steak tips with noodles and corn, then crusted cod with breadcrumbs (cooked in butter) served with brown sugar glazed carrots and a potato with butter, and last night I had 2 hot dogs (with hotdog rolls) and with salted fries on the side.
- Exercise habits: enjoys lifting hand weights/sit-ups/push-ups every other day for about an hour, daily walks with the dog that average about 1 mile
- 24-hour recall: 3 cups of hot coffee - each contained half and half with 4 Splenda, a blueberry muffin, 2 hot dogs in rolls with salted fries, handful of pepperoni slices and green olives, baloney and cheese sandwich on white bread with a handful of cape cod potato chips.
PES statement is a framwork to develop a nutritional diagnosis , it includes the problem, etiology and signs and symptoms
P= overweight related to unable to maintain diet change.... This is evidenced from (Weight history: has fluctuated a little over years, but has consistently stayed around 170 the last 5 years)
E = Decreased activity level and increased intake secondary to diabetes (as pt is taking actos)
S= evidenced from the pts weight and BMI and family history of obesity
