1. A support worker should work to improve the quality of the client’s life and act with empathy while rendering the services. Two ways a support worker could ensure that they accurately interpret an individual plan are:

a. Client should be the focus of care and care should be delivered to meet the client’s needs mentioned in the individual plan. They should honor the client’s rights including the right to confidentiality and think and do what is better for them. They should approach the client only after studying the individual plan and get regular feedbacks from the client regarding the individual plan based care. They should always seek methods to improve the care as per the client requirement.

b. Regular feedbacks and opinion of family members and healthcare professionals should also be considered and any changes if necessary should be made according to the individual care plan. Family members and healthcare professionals should make sure the support worker is getting individual-plan-based proper and timely instructions from them.

2. Two reasons a support worker should read and interpret individual goals are:

a. To deliver client-requirement-based support.

b. To improve client’s quality of life and functionality.
