Information about VANCOMYCIN:-
Generic name :- Vancomycin
Brand name :- Vancocin/ Amoxil
Type of drug :- Glycopeptides Antibiotics
Pregnancy category :- Category B( Oral) Category C( Parenteral )
Therapeutic action :- Inhibits bacterial RNA and cell wall synthesis; alters permeability of bacterial membrane; causing cell wall lysis and cell death.
Indications :- Helps to treat resistance Staphylococci infections, pseudomembranous colitis, Staphylococcal enterocolitis, endocarditis, prophylaxis for dental procedures, diphtheroid endocarditis.
Contraindication :-
- Hypersensitivity to drug or corn if given with dextrose solution
- Renal impairment
- Hearing impairment.
Dose :-
Adult :- 500mg to 1g PO or IV q6h for 7-10 days
Pediatric :- 40mg/kg/d PO or IV in four divided doses
Do not exceed 2g/d
Adverse effects :-
Fever, pain, hypotension, thrombophlebitis at injection site, ototoxicity, tinnnitus, nausea, nephrotoxicity, leukopenia, neutropenia, eosinophilia, dyspnea, wheezing, Red man syndrome, Anaphylaxis, Chills, Superinfection.
Interactions :- Drug interaction may cause additive ototoxicity and nephrotoxicity with other ototoxic and nephrotoxic drugs ( Aspirin, Aminoglycosides, Cyclosporine, Cisplatin, loop diuretics)
Nursing consideration in implementation and Patient Education :-
Nursing consideration :-
1) Must monitor blood level to ensure therapeutic peak and trough levels and prevent toxicity.
2) Should be infused over 60 minutes
3) Rapid infusions may cause hypotension
4) Monitor IV site closely
Patient Education :-
- Advise patients on oral Vancomycin to take as directed. Take missed doses as soon as remembered unless almost time for next dose; do not double dose.
- Instruct patient to report signs of hypersensitivity, tinnitus, vertigo or hearing loss.
- Advise patient to notify health care professional if no improvement is seen in a few days.
- Patients with a history of Rheumatic heart disease or valve replacement need to be taught importance of using antimicrobial prophylaxis prior to invasive dental or medical procedures.
