Answer. Client presenting lodges and difficult delivery.

Involution of uterus is a normal physiological process, it begins immediately after delivery. Involution of uterus depends amount of uterine contraction, expulsion of lochia or discharge and endometrial regeneration.

In primipara involution takes place smoothly than multipara. Multipara women have relaxed uterine muscles.

If any placental pieces are lodges in the uterus, it prevents uterine contraction and involution.

In client with difficult delivery like multiple pregnancy, prolonged labour, precipitating labour, client with GDM, assisted delivery will affect uterine muscles integrity and cause prolonged involution or impaired the process leads to post partum hemorrhage.
