INTRODUCTION TO PATIENT AND FAMILY:

DEVELOPMENTAL/MEDICAL HISTORY: History should include immunization status, previous infections, contact with infected person, symptoms that the child exhibited, allergy, previous neurosurgical procedures, any recent antibiotics.

HOW WOULD YOU OBTAIN VITAL SIGNS, HEIGHT, AND WEIGHT? : A nurse should approach the patient with required PPE ( personal protective equipments) for any procedure.Better to use the disposible item such as electrodes, thermometer probe and disposible BP cuff, disposible bed cover. he wont be able to stand for checking height and weight, so consider recumbant height( substract 0.8cm from recumbant height to get the actual height), ask the mother to hold the baby and take weight, then she has to weigh alone, substract the latter from former weight.
