Importance of Assessment, diagnosis, planning , implementation and evaluation in Health care and Nursing process:
Healthcare is the treatment, management and prevention of illness or the preservation of mental and physical well-being through the services offered by the professions like medical, nursing and allied health professions. Nursing process is systematic goal orientated process for client centered care that consists of five sequential steps ; Assessment , Diagnosis, planning, Implementation and evaluation. All these five steps are essential and important not only for nursing process but also for Health care.
Assessment
The act of assessing or appraising a client's condition , environmental situation or , quality of Healthcare services provided. Assessment is the basic step of nursing process, by which the nurse can gather subjective and objective data through Data collection process, Health Assessment; that includes physical examination, history collection etc.
Diagnosis
It is the process of identifying the nature and cause of illness or identifying the patient's problems or needs by the help of data , gathered during assessment phase. Medical diagnosis of illness or Nursing Diagnosis regarding client's problems are formulated in this stage. Without determining the problem, no health professional can progress to the next step
Planning
In the planning process, the health care professional can set the goals or expected outcome and can formulate the course of action to solve the client's problems or to achieve the expected outcomes . The goals should be feasible and measurable. Without a proper planning, it will be difficult to achieve the goals. Nursing Interventions are the result of planning process.
Implementation
The process of moving an idea from concept to reality is called Implementation. The Health care goals or goals of Nursing process can only be achieved through the proper implementation of the interventions , that are formulated in the planning phase.
Evaluation
It is the final step of any course of action in Health care and in nursing process. In this step nurse or the evaluator conducts evaluation to determine whether the expected outcomes are achieved or not. Through the evaluation process one can understand the effectiveness of the interventions applied. It act as an important step in Evidence Based practice. For example: A nurse can evaluate the effectiveness of tepid sponging in reducing the fever, or A nurse manager can evaluate the patient satisfaction by conducting the surveys or administering the rating scale and analysing the results.
