Explain why a ACE I is considered highly beneficial in patients with mild to moderate renal failure, especially diabetics, and detrimental in patients in renal failure or have renal artery stenosis?

ACE I is beneficial because it reduce systemic vascular resistance and lower glomerular pressure.It is used as antihypertensives.It suppress Monocyte Chemoattractant protein-1(MCP-1) and there by reduce the chance of diabetic nephropathy among diabetic patients.

ACE I is contraindicated in renal failure because it cause hyperkalemia and sudden death among patient with falling GFR.It is also contraindicated in Renal artery stenosis due to risk of azotemia and impairment of glomerular perfusion.
