I need ideas!!! You are discharging your patient home with a g-tube. What education/teaching will you provide to your patient and their family? Identify any 2 key problems associated with g-tube feeding and create a care map based on these problems, one nursing diagnosis for each key problem. Key Problem: ND: Related to: As Evidenced by: Supporting Data: Outcome: Interventions: Teaching: Thank you in advance!.
1) A gastrostomy tube ( g-tube) is a tube inserted through the abdomen that delivers nutrition directly to the stomach. G- tube include an incision in the patient's epigatrium as part of a formal operation.
Discharge instructions we will provide to patients and their family are as follows :-
- Ask the patients to wash the hands throughly with soap and warm water before starting feeding.
- Before feeding begins, healthcare provider may have check to see that stomach is empty.
- Clean the area around the tube with mild soap and water.
- Put the area dry after bathing.
- Clean the area more often if it gets wet or if it is leaking some discharge.
- Keep the disk( flange) a few millimeters off the skin. This should leave just enough room for a gauze sponge.Pulling the flange too tightly can damage the skin. But leaving the flange too loose leads to leaking around the G-tube.
