Percussion in the lung field can be done by keeping the middle
finger firmly on the chest wall along with intercostal space and
tap chest over the distal interphalangeal joint with the middle
finger of the opposite hand. tapping movement should come from the
wrist. tap 2-3times in a row. never leave the percussing finger on
chest otherwise you will lose the sound. stand on one side and with
your flat of the hand and tap from top to bottom of the chest from
side to side to compare the sound.
2, AP diameter is the anteroposterior diameter that normally
smaller than the lateral diameter. shoulder and scapula will be at
an equal horizontal portion. the ratio of AP vs transverse diameter
is 1:2.
3, Percussion the same in the areas of both sides including apices,
posterior, lateral and anterior of the chest at about 5cm interval
from upper to lower chest.moving from left to right and right to
left and both sides should be compared for dulness and resonance
inside the chest wall.
4, listen the cough carefully for duraito ,coarse, triggers, dry or
productive cough and associated shortness of breath, wheeze etc.
look for signs when coughing like sweating, clubbing, cyanosis and
respiratory rate, crackles, wheeze,plural rub etc. careful history
collection and complete physical examination find out the presence
of cough.
