SBAR note :-

S- The patient came to the emergency with signs and symptoms of alcohol withdrawal.

B - Patient is a heavy drinker and he have been kept away from alcohol which has resulted in withdrawal symptoms

A- On assessment he had the following signs and symptoms :-

shakiness, sweating, or loss of appetite,agitation, restlessness, or irritability,nausea or vomiting

R - The patient is being shifted to ward for intensive care and to start anxiolytics / sedatives as per the advice
