On admission Shamsa was asked by the nurse on duty to rate her pain using the pain scale of 1-10 and she said that it is 7. Her vital signs were also taken and recorded as follows: Temp: 37.2 C; RR: 22 b/min.; PR: 80beats/min; Bp: 135/84 mmHg. After thorough assessment by the doctor Shamsa was diagnosed of having Peptic ulcer. U

  • Assess the level of the pain ,to plan for care
  • Administer antacids ,proton pump inhibitor as per order to prevent acid secretions and relieve pain
  • Provide a bland ,soft diet for easy digestion and absorption gastric irritation
  • Encourage to take milk ,butter milk to soothe the gastrointestinal tract

Evaluation

The patient will be relieved of pain and able to eat a normal diet
