1, Discharge summary should contain the reason for admission,
significant findings, procedure, treatment provided, condition of
the patient while discharge, patient and family education,
signature of the physician, medications, appointments, followup
plans, test results, pending lab work, etc.
2, crutches provide support and balance when you walk. proper
crutch fitting important for minimizing the pressure and weight.
evaluate the patient's understanding of learning about crutch use.
ask patient questions and feedback related to the understanding of
crutches. ask the patient to restate the crutch use for mobility
and immobility use in own words. Patients need reinforcement of
re-teaching if they state wrong instructions.
3, advise the patient to wash hands before and after using oxygen
and make sure the hands are dry before touching the oxygen
unit.
pressurize the oxygen take, make sure oxygen gauge on the tanks are
full enough.
check the water level in the humidifier bottle.
advise the patient to check the tube position under nasal
cannula.
Face maks should be places on the nose and mouth
keep the o2 cylinder at least 3meters away from open flames.
never smoke near to oxygen equirpment.
keep the oxygen equipment at chest level and it should be clean and
dry.
Do not cover the o2 cylinder with cloth to plastic bags keep the
empty cylinder away from full clinder and make the cylinder is full
always and keep the backup oxygen cylinder ready always.
