Weight of the child = 44lbs = 20 kg ( 1kg = 2.2lbs)

Recommended dose = 15mg/ kg/ day

available concentraation of medication = 125mg/ 5 mL

To check whether the ordered dose is safe

Recomended dose per day15mg x 20 kg = 300mg

So the ordered dose and recommended doe are the same which indicates it is a safe dosage.

The formula for dosage calculation is

(D) Desired dose ( dose ordered) x (Q) Quantity of available dose = dose to be administered

(H) Dose in hand ( available dose)

300 mg x 5mL = 12 mL

125 mg

12mL = 300 mg

6ml = 150 mg

Thus  Biaxin 2.5 gms granules oral suspension, 125 mg/ 5 mL 12 mL to be given daily . 6 mL each time as it is ordered twice daily.
