Weight of the patient = 40 lb 10 oz

40 lb = 402.2 =18.18 kg

10 oz = 10 x 28.3 =283 g = 283 1000 = 0.283 kg

40 lb 10 oz =18.18 kg + 0.283 kg =18.463 kg

Available dose =75 mg/ml

Amount of clindamycin to be given = Prescribed dose Available dose x Volume of drug

=(15075) x 1

= 2ml

Recommended dose = 8 -25 mg/kg/day

Recommended dose for 18.463 kg patient = 8 x 18.463 kg to 25 x 18.463 kg =147.704 to 461.575 mg/kg/day in three or four divided doses

As the patient is receiving 150 mg PO every 6 hours ,patient is receiving 150 x 6 = 900 mg of clindamycin in a day which is not safe
