Here the situation is a pediatric patient of 4 year old complaining of pain. The diagnosis in this case can be the following :

Insect bite

Constipation

Intususception

Gastritis

Gastroenteritis

Appendicitis

So in this case it is very difficult to reach a diagnosis just by examining the patient especially in pediatric age group. So the first thing here is to relieve the pain. Any antispasmodic -analgesic combination can be given. Reassure the parent and advise to get a pediatric consultation.
