While preparing to give medications, a nursing student checks the electronic health record, completes all steps in the safe medication administration process and completes the prescribed double and triple checks with the medications and the patient. Medications to be given include a daily vitamin, an anti-hypertensive, and an anti-diabetic agent. The nursing instructor reviews the medications and approves administration as well. After administering the medication, the instructor and student go to the electronic health record to document administration. Once at the computer, the nurse for that patient approaches and states that she administered all the medications already, but did not have time to document all the medications. The result is that the patient received two doses of all the medications within one hour.

1, The double dose of the medications adversly affects the patients health,antidiabetic agents and antihypertensive agents causes hypoglycemia,giddiness,hypotension etc,when we are giving medications like antihypertensives and anti diabetics we should confirm to the ward assiigned nurse to avoid errors.
