The patient is here today to follow up on his chronic renal failure. He is a 59-year-old with multiple medical problems including complicating diabetes with severe neuropathy and idiopathic dilated cardiomyopathy. Laboratory today: Glucose 224, sodium 139, potassium 4, chloride 96, carbon dioxide 34, urea nitrogen 4, creatinine 2.2, total calcium 8.9, and phosphorus 3.2. The patient needs to continue current treatment. Also reemphasized the salt restriction to 2 grams per day and moderation in protein intake to 0.6 gram per kilogram per day.
This patients cardiomyopathy treatmetn is the first priority,Treatment of dialated heart chambers aimed at decreasing the size and the substance in the blood stream that enlarge the heart and ultimatly lead to worsened symptoms,the medications for the cardiomyopathy are ACE inhibitors,Diuritics,and betablockers.
The chronic renal failure treatment depends on the severity of the symptoms and the blood values, in this patients laborotory values shows that she can be treated with medications dialysis is not needed,the medications commonly used are Diuritics,Calcium reducers, Bone marrow stimulants and dietary suppliments and the salt restricted diet.
