Things that a nurse do prior assessment includes

  1. Introduce herself or himself :- this helps to maintain a good rapport with the patient and makes the patient comfortable. Also helps to make trust in patient. Also it includes how they wish to be addressed etc
  2. Confirm patient by asking the name and age
  3. Arrange necessary articles needed for assessment and explain briefly the things that you are going to do. Also observe the environment
  4. Provide proper position to patient and also do hand hygiene.
