docs order: 600 mg q 6 hrs. Patients weight: 14 kg Safe Dose Range: 50-200 mg/kg/24 hours The drug should be diluted to 20 mg/mL and infused over 1 hour Vial on hand: 1000mg/5cc What is the fractional dose? ____ mL/dose
First of all we will Validate ordered dose to make sure it falls within the therapeutic range:
Patients weight: 14 kg
The recommended dose is 50-200mg/kg/day divided q6h, so we will calculate the minimum and maximum dosages.
Minimum Dose 50mg x 14kg = 700mg/kg/day divided q6h 175 mg per dose |
Maximum Dose 200mg x 14kg = 2800mg/kg/day divided q6h 700mg per dose |
The child would receive 600mg/6hr = 2400 mg/24hr or 2400 mg/day. Therefore, the prescribed dose is within the therapeutic range. (Less than the maximum value i.e 2800mg)
Drug Dilution = 20mg/mL
Ordered drug per dose = 600 mg/dose
Volume of drug that need to be infused or the fractional dose = 600mg/dose/20mg/mL = 30mL/dose.
