1. Change glucerna to ensure as glucerna mainly for those with diabetes. If it is given long term to those without diabetes , it may lead to inadequate nutrition to body. Also lack proteins for functions. Ensure is a complete nutritious diet which can be used for patients. So advise to give ensure after discharge.

2. Osmolite 1.2 change to ensure or vital AF 1.2

Osmolyte 1.2 is a high protein diet. If given continuous ,it cause protein overload and lead to biliary overload and hepatic dysfunction. So the changes in stool colour occur.
