As per the guidelines, effective communication is one of the major tool for a good handover. With poor communication many errors ahve occured including medication administration error. So, Based on the SBAR tool, transferring responsibility and accountability of patient care during a patient transition between different health care providers can be planned accordingly.Various resourecs say that to have a good hand over we should be organized. Always focus on the priority porblems rather taking some other information about the patient. Based on the SBAR, a handing over checklist tool can be prepared so that based on each categories proper handing over can be given.Keep hand over planned and avoid repetition. and finally leave enogh time for queries.

Using SBAR tool we can assess the situation, background, assessment and recommnedation. Based ont his we can provide a beter handing over to the next shift health care providers.With the SBAR tool, we can assess why we have called physician and what is the situation.What is the background information for a particular patient or the tretament behind that.what is the current problem identified through the assessment of the problem and how it can be corrected. All theses can be answered with the help of SBAR tool.
