The process needed to assist a person accessing the community
options outlined in their individual plans are:
- the first step is to set a good relationship with community
members and clients.
- Then share your feelings and get his feelings and understand
his thoughts, cultural differences and beliefs, and values.
- Identify community and assess the need of client.
- Assess the eligibility criteria to access the services and
requirements, do paperwork.
- The second step is to make sure the active participation of
client in his decision making and assess his strength and weakness,
capacity, and capability of client.
- Third, set goals with the client.
- Then, strategies should be set clearly with client and also
assess the budget of making individualized plans or resources
needed in achieving goals.
- Identify the limitations and barriers that can come in a way,
for example stigma, communication problem, etc
- Then make a plan and implement the strategies, get help from
support systems and groups.
- Then evaluate and do the follow-up.
- Even, you can help a client by providing education, training
program, and demonstration to access the community option. As
health education in the community is an integral part.
ADVOCATE SHOULD HAVE SKILLS TO OVERCOME THE BARRIERS IN
ACCESSING THE COMMUNITY OPTIONS, DIFFERENT BARRIERS CAN BE:
- lack of knowledge
- stigma
- lack of support
- lack of participation
- cultural differences
- communication barrier
- physical health
- less motivation etc.

- Good teaching strategies
- critical thinking
- good leader
- respect all differences etc
