Describe a health model to assist in creating a plan of action for the family with a 41 year old man at risk for diabetes and hypertension, 31 year old wife, and 3 children aged 9 years with overweight and eye problem, 5year girl and 6 months old baby girl

Health model:

First of all man is the head of the family know a days our medical fealed have developed in high.Though there is medication,due to these he can control his diabetes and hypertension with medication, diet and exercise

And for children if they are over weight try to provide balanced diet. Make them to do exercises ,try to avoid intake of junk food and fast readymade food.These will maintain our BMl.

If you follow these you can bring back your health as well as your eye sight.so you can enjoy your life along with your family and be healthy.
