Mrs. Smith is a 75 year old widow who is very active and healthy. She was brought in to the ED after collapsing at the grocery store. Mrs. Smith explains to the nurse practitioner she doesn't know what happened but she has been feeling exhausted and no energy for the last month. She tells the nurse practitioner, " I must be getting old, I am slowing down." The nurse practitioner is suspecting there is more going on than just age.

Certain examinations should be done by the nurse to get data

First and formost thing a nurse must do is checking vital sign.

  • Blood prssure
  • pulse
  • respirstory rate
  • oxygen saturation

Physical examinations;Head to foot examination including,

  • inspection
  • palpation
  • percussion
  • auscultation...will help nurse to assess the health status of the patient.

The nurse has to explain the patiet about care plan like all procedures that are going to be performed including diagnostic procedures,medicine administrations and other treatment strategies.

  • if requires an ECG has to be taken followed by Ecocardiogram if there is any change in heart rythm
  • Blood test including total blood count,blood sugar,cholesterol level,blood urea ,creatinine,BT,CT,troponin test if there is any change in ECG has to be performed.
  • xray and other diagnostic methods willalso be performed as per the order of the physician.

Getting informations before diagnostic procedure;

allergic to any medication

past medical or surgical history

family history of any diseases

possibility to get any test reports if done anything earlier.

These are the assessments ,patient education and personel data collection should be done by a nurse.
