Medication: Carmustine (BICNU)

Category Class: Alkylating Agents

Expected Pharmacological Action: Inhibition of DNA synthesis, Protein synthesis,

Therapeutic Use: To inhibit the growth of cancer cells and used in multiple myeloma, brain tumor, Hodgkin's disease, non-Hodgkin's lymphoma, Malignant glioma, Glioblastoma Multiforme.

Complications: Nausea and vomiting with burning sensation at the site of injection, Chances of skin problem and fall in total blood count, bone marrow suppression, inflammation of the lungs and altered kidney function.

Contraindications/Precautions: Bad infection, Low platelets and WBC, pulmonary fibrosis, anemia and leukemia, liver problems, decreased kidney function, pregnant or breastfeeding mother.

Interactions: Drug interaction- Thiotepa, Zarxio, Neopogen, Lomustin, Carboplatin Novaplus.

Disease interaction- pulmonary infiltrates/fibrosis, infections, myelosuppression, renal dysfunction and hepatic dysfunction

Medication Administration: Intravenous, according to the weight and height of the pateint

Nursing Interventions: Prior administration of an antiemetic to reduce adverse effect and monitor for nausea and vomiting up for 6 hours. Also timely monitoring of lung function, total blood count, liver and kidney function.

Patient Education: Educate them about the use, side effects, uncomfort and uneasiness and safety. Also talk about the severity of side effects and the effectiveness of the medication.

Evaluation of medication effectiveness: Yes, it is considered effectiven and affordable medical solution since it high lipid solubility and the relative lack of ionization at physiological pH, enables it cross the blood-brain barrier quite effectively.
