A 90-year old male resident with dementia has been living in a long term care home for several years. The family decides and requested for DNR (Do Not Resuscitate) as an End-of-life care decision considering the best interest of the client. The evening shift documented that the family requested for the client status to DNR in the clinical notes but forgot to endorse to the upcoming shift. They even forgot to document the information in the 24-hour report or to notify the physician. The family was so caring to the client and went home to shower and eat after informing the staff on duty. Upon return, the family saw that the staff is providing CPR followed by resuscitation and now he is in ventilator in a vegetative state. The family was desperate and considering legal action. What the ethical issues and dilemas in this scenario?
