Mr Ali is drowsy with periods of confusions impaired short-term memory—remembers zero of three objects after 1 minute. He appears to have left visual field loss. He has right lower facial weakness left tongue deviation. His muscle bulk is symmetrical, tone slightly increased on left arm/leg. He has diminished attention to objects and people on the right side of bed.

- Since his right side had the weakness , don't keep any things left side of him .

- Keep the things infront him

- Orient him through calendar ,clock , images , pictures and family members from front

- Facility of proper light and air

- Avoid bright light to avoid strain

- Keeping the floor dry to avoid fall

- Raising the side rails of the bed to avoid fall

- Using the washrooms and toilet which have side rails to hold

- Never leave the patient alone

- Transferring the patient from right side
