Nursing interventions & Rationale:

- Assessment of the general condition- Provides baseline data to plan care.

- Put wrist band for such patients eg. If diabetic then sudden hypoglycemia may also cause it- to identify them.

- Orient the patients to the environment. Call bell within patients reach. All the necessary items within patients reach- To formulize the patient so that it will help them to get to know the surrounding easily. Also prevents patient to get out of the bed without assistance.

- Administer medications as per doctors order.

- Always assist geriatric patients- If geriatrics are left alone, dizziness can cause risk of fall.

- Bed height low & side rails always up- safety precautions.

- Encourage patients to move slowly & perform tasks at own pace- sudden movement can trigger dizziness.
