A positive ramification of a discounted fee-for-service arrangement is that physician's or doctors are paid to provide you with a treatment, and so may be more likely to offer you a treatment than if they were paid a standard monthly usage.

Negative ramifications of discounted fee for service arrangements is that preventative care is not covered. This affects not only the physician but more the patient. Using an out of network provider would result in higher out of pocket cost for the patient. The reason why is because any unnecessary test or exams would be an out of pocket expense for the patient.
