The vital signs include the

Blood pressure write is as systolic /diastolic mmHg (eg 140/90 mmHg)

Heart rate - Check the pulse mainly at the radial site and count it for one full minute and document it like the rate ,relular or irregular and the nature of the pulse( 0- pulse is absent,1+ - pulse is weak,2+ - pulse is normal,3+ - pulse is bounding)(eg 82 per minute ,regular,2+)

Temperature - wile documenting document the unit also like whether it is degree celcious (0C) or Fahrenheit alaso mention the correct place also like orls temperature,axillary temperature or forehead temperature etc

Respiration - record the respiration for one minute (eg 18 breaths per minute)

Pain - I fit is neumerical scale write the pain score like 4/10 or 5/10 ( just entioned the values as example)

Oxygen saturation document it like 100% in room air or 95% with 5liters of oxygen
