#. First of all I would do a fall risk assessment :-
Step 1: Identify hazards, i.e. anything that may cause harm.
Step 2: Decide who may be harmed, and how.
Step 3: Assess the risks and take action.
Step 4: Make a record of the findings.
Step 5: Review the risk assessment.
#. Actions to be taken to prevent fall include :-
Proper patient assistance and orientation
Eliminate slipping and tripping hazards.
Keep the bed at the proper height during transfer and when the patient rises to a standing position.
Don’t keep the bed in a low position at all times.
Check chairs, toilets, and safety grab bars for potential safety problems.
Use proper room lighting.
Make sure the patient wears proper footwear
