If you have lost your personal health care card, or it has been damaged, call the AHCIP office to order a replacement card

Procedure:-
Albetans can request replacement card by telephone, in writing or in person.

Eligibility:-
An owner of a health care insurance plan card.

Processing time:-
If you visit an authorized registery agent or telephone the AHCIP, a replacement card is generally received with in 1 to 2 weeks. If you mail or fax Alberta Health with a request, a replacement card is generally received within 4 to 6 week.
