Respected Sir ,

I am writing you concerning a recent occupation injury that has occurred in our division of welding department in which a worker has lost his life due to the slip of the heavy portion of the machine on him which is used for welding , because of its improper maintenance and loose functioning .

The worker was immediately taken to the hospital but we can't save him . It's a loss to our organization and to his family . To avoid this in future I request you to kindly implement survey process of heavy machines , about their functionality , safe working environment etc . I hope that you will take action towards it as soon as possible .
