1)ASSESSMENT.

a. Nurse should assess the anxiety level of the patient.

b. Nurse should assess any physical or physiological abnormalities related to anxiety.

c.Nurse should collect physical, psychological, social data.

2)NURSING DIAGNOSIS.

• Anxiety related to real or perceived threat to biological integrity or self concept, as evidenced by various physical and psychological manifestations.

•Powerlessness related to impaired cognition, evidenced by verbal expression of lack of control over life situation and unable to decision making related to own care.

3.patgophysiology.

Due to Etiology, hypothalamus will be stimulated as a result it increases cortisol , epinephrine release. Stress hormone will be activated. Anxiety is produced.

4) Goals.

• For 1st diagnosis-

a. Short term goal is - Patient can identify the symptoms of anxiety.

b. Long term goal is - patient can control his anxiety before reaching panic level in future.

•For 2nd diagnosis-

a. Short term goal is - patient's self esteem will be improved.

b. Long term goal is- patient's can solve his problem and make decision independently.

5. Planning.

•Anxiety level of the patient should be assessed.

• Patient should be talked in polite manner.

•patient should be assisted to increase self esteem.

•patient should be helped to identify life situation that are within patient's control.

6. Intervention.

• For 1st diagnosis.

1)Low stimulus environment is provided to the patient.

2) Tranquilliser is administered as per doctor's order.

• 2nd diagnosis.

1)Deep breathing exercises is taught to the patient.

2) patient is allowed to take as much responsibility as possible in self care.

7) Rationale.

• For 1st diagnosis.

1)A low stimuli environment may decrease anxiety.

2) Tranquilliser helps to relief from Anxiety.

• 2nd diagnosis.

1) to reduce anxiety.

2) to increase patient's feelings of control.

8.Evaluation.

•Anxiety level will be decreased.

• Increase self esteem of patient.
