Differences between autologous graft performed in situ vs reversed are as follows;
1) In situ bypass requires an intact ipsilateral saphenous vein. The procedure cannot be applied to increasingly large number of patients in whom the saphenous vein is absent or has been interrupted. On the other hand reversed vein bypass can be applied to all patients who require lower extremity bypass.
2) In situ bypass grafts have a higher rate of requiring a secondary operations to maintain its patency than that of the reversed graft series.
