The main purpose of assessment is establishing a database; concerning a client’s physical, psychosocial, and emotional health for identifying health-promoting behaviors as well as actual and/or potential health problems. With the help of patient history, physical examination, and diagnostic tests we can analyze assessment data to identify the nursing diagnosis.
