He is oriented to person only .He is unable to give a medical history. He says he lives with his wife Leila.According to old records Mr Dula has a daughter Autumn who has been called and has just arrived. She states that he lives alone. His wife died 5 years ago.Autmn states that Mr Dula has been increasingly forgetful over the past few years but she thought he was doing fine at home. He is taking multiple medications for hypertension and seasonal allergies. His daughter has been filling a pill planner once a week. Mr Dula often forgets his medications or is a day or two ahead in the pill planner. He has not been driving for about 4 years. Today is Wednesday. The ED physician has made a new diagnosis of dementia   __ Alzheimers type.
