Soap note is a methods of documentation employed by health care providers to write out notes in a patient chart ,along with other common formats , such as the admission note.The four headings of a soap is subjective, objectives, assessment and plan.

1 what are the clinical features present right now?

2 what you should feel or experience right now?

3 Identify the main problem must occur to perform effective and efficient daignosis ?

4 About history of present illness,which includes, * when did the chief complaint begin?

* Where is that Chief complaints located?

* How long has the chief complaint been going on for?

* How does the Patient describe the chief complaint?

* What makes the chief complaint better

* Does the chief complaint move or stay in one lication?

* Is the chief complaint work at certain time of the day?

5 History about,medical history ( pertinent current or past medical condition), surgical history (try to include the year of surgery and a surgeon if possible), social history (education, employment,eating,sucide , depression)

6 Do you experience decreasing appetite?

7 How many weight you have?

8 Do you feeling any abdominal pain , hematochezia?

9 Do you have any allergies reaction towards medications?
