Nursing Intervention and Rationale

a)find out the understanding of current situation and methods of coping with problems in life-Identifies coping skills to be used in current situation.

b)Encourage verbalization of feelings-May help the patient to find solutions for long unresolved issues.

c)Encourage patient to learn relaxation skills,guided imagery and visualisation-Helps patient to relax and cope with stress

d)use peer support to deal with drug hunger-self help groups are effective in promoting abstinence in group member.

e)administer medication as prescribed-

2.low self esteem related to social stigma attached to substance abuse ad evidenced by verbalization shame and guilt

verbalization of acceptance of self as is and increase sense of worth

Nursing intervention and rationale

a)Encourage verbalization of individual situation-patient has difficulty to accept the depth of drug's influence in present situation

b)provide reinforcement of positive actions-helps to accept individual to identify self with positive attributes

c)Encourage patient to identify substance abuse itself as a problem and problems can be dealt without drugs-Understanding drugs are problems than solution help them to refrain from it
