d) Asess the client s bladder for urinary retention
Reason-The primary manifestation of urinary retention is a distended bladder or an inability to empty the bladder completely ,The retained urine increase the hydrostatic pressure against the bladder wall which result in hypertrophy of the detrusor muscle. After assessing the patient for urinary retention ,the catheterization should be done to drain the urine The dark amber urine indicates the dehydration. it will not cause abdominal distention.
