Jackie Wilson is a junior nursing student who was assigned on Tuesday to her first day of operating room (OR) observation. The OR staff was excited that Jackie was able to see one of the transplants on a day when several transplants were performed. Later that evening Jackie shared with her family how interesting it was to see the transplant and to see the staff work so well together and function as a high-performing team. She also shared many details about the surgery because, of course, the staff had been sharing these details with her.
Three days later, the academic health center released a press briefing to media outlets in the region reporting that Tuesday had been a record day for transplants at the academic health center. That morning at work, Jackie’s dad told his colleagues that his daughter had been in the OR for one of these transplants and proceeded to communicate many of the details that Jackie had shared at the family dinner table.
Case Analysis
One of the lapses in professional identity that appears in the research literature is that of breach of confidentiality. That is certainly the main problem in this case. However, there are other influences that more fully tell the story—the inclusive approach that health care providers had for a new learner, the enthusiasm that faculty have for what students are learning, and the genuine interest in being part of something so intriguing. What is difficult to understand is where and how to draw the line in a situation like this—for the student, for the OR staff, and even for the faculty. Students hear clinical examples in class and discuss specific details of their patients’ circumstances in clinical conferences. Of course, all students sign confidentiality agreements—but do they really understand?
This is a situation where a rule-based approach is probably not as helpful as helping the students build their own understanding of professional identity as it relates to confidentiality. Being more reflective and conversational with the students would probably yield better outcomes in the long run. A combination of specific examples of what NOT to do along with conversation about other possible scenarios would better prepare novice professionals for the complex system expectations that exist in health care today.
